Complete application and return to school office. Set up an appointment for a placement test. The $50 application and testing fee is to be paid no later than the day of the test (This fee is returned if we are not able to place the student).
We will schedule an interview with the student, parent(s) and principal to go over the test results and determine placement of the student.
We will contact you with your child's acceptance or denial. $150 non-refundable Reservation and Registration fee is due within three days of acceptance in order to reserve your child's seat.
The Classroom Enhancement Fee is due July 1st.
Complete the required items. Please use the following bulleted items to ensure all documents are sent to our office in a timely manner. (Acceptance cannot be finalized until all of the following items are received in our office.) Also, please have your pastor or minister fill out the Pastoral or Ministry Leader Recommendation (Pastor/Ministry Leader may fax it directly to us at: 503-257-9284).
Crossroads Church may be able to provide pastor recommendations.
Copy of student's birth certificate. · Original immunization card (if the student has not attended school before); · Signed request for records form (to be sent to previous school to obtain recent report cards and immunization card); · SAT scores and IEP (if applicable) |
