CROSSROADS CHRISTIAN SCHOOL Student and Parent Sports Agreement
RULES AND GUIDELINESRULES AND GUIDELINES RULES AND GUIDELINES1. Team members represent both the school and Christ and are, therefore, expected to uphold the highest standard. This is especially true in the area of attitude (i.e.; having a good one) and effort (always doing one’s best). This applies on and off the court. In a similar manner, PARENTS are also representatives, and as such, are expected to exhibit the standards of the school in behavior and attitude, especially when dealing with opposing teams, officials and other fans. The coach may dismiss players from the team at his discretion at any time for problems with attitude. 2. Academics are first priority. Teachers and parents will be in communication with the coach on how players are doing academically. See “Academic Policy” on the reverse side of this form. 3. Each player must have a current sports physical before the first practice. Physicals are valid for two years. 4. The sports fee of $75.00 must be paid to the business manager/office before the first practice. A player may not play until money and forms are turned in to the school. 5. All players are expected to ride in the van to away games. If the coach is notified in advance by a player’s parent, then the player may ride home from an away game with a parent-authorized adult. 6. Each player is expected to take proper care of their body—including proper rest, nutrition and medical care, as needed. 7. For health concerns, each player will need to provide their own sports bottle with their name clearly marked. 8. Each player is expected to arrive on time and attend each practice and game. Exceptions will include the following: Illness with parental notification to the office, Doctor’s appointments (please notify the coach in advance), and family emergencies. 9. Missing practice even for the above reasons may result in lost playing time. 10. If a player is not present at school on a game day, he will not play in that game. One unexcused absence will result in the player not participating in the next game following the absence. Two unexcused absences will result in the player’s dismissal from the team. 11. Players are required to come to practice in modest shorts and T-shirt, and court (tennis or basketball) shoes (no sweats). Players must wear laundered, well-kept specified team uniform for all games. For volleyball - kneepads are required at all times. 12. Headbands, wristbands, jewelry and other accessories are not permitted during practices or games. 13. Parents are responsible for picking up players on time after practice and games. Any players not picked up by the end of practice/games will be sent to Shepherd Care and parents will be charged accordingly.
Please return this form by the first practice.
